I figured I'd post this since school will be starting in a few weeks. If you want your child to be exempt you have to fill out the exemption form and get it notarized. Many years ago when I got the form notarized for dd I just called someone out of the yellow pages and it cost me $25. For ds's form, I knew someone who worked for a lawyer and the lawyer signed it for free for me.

I gave the form to the school and got a mailing from the Health Unit and I mailed them a copy and never had any issues. When dd was in JK there was a suspected case of rubella in the school and she had to stay out of school until it was confirmed it was not rubella, as did all children her were not up-to-date on their MMR vaccine, but otherwise it was fine.
