REPORTING PROCEDURE FOR ANY MEMBER(s) FOR A FORMAL COMPLAINT:

Member should use THIS Formal Complaint Form below.


PM (PRIVATE Message) the Formal Complaint Form to:

1. Affected Member(s)
2. Moderator(s) of Forum
3. CC (PM Copy) to a Forum Administrator or to Owners (elainesj or Mr Mom) if Admin is involved.

1. Member Name(s) for whom this complaint is being made against:

2. Reason(s) behind the complaint.

3. Evidence that you have to support your claim: (Please attach and/ or forward any documentation (PMs) that pertain to problems you may have been dealing with surrounding this issue)

4. Rules/ Regulations/ Mission Statements that you feel have been violated (Please refer to the Guidelines Forum for specifics):
